Job Summary

The MSW provides medical social work services to Hospice patients and their family as a part of the interdisciplinary care team. Addresses the biopsychosocial needs and concerns through assessment, counseling, resource development and advocacy. Assesses for pre-death grief and bereavement needs of the patient and family. Provides support, education and consultation to the interdisciplinary team. As the Clinical Lead, the MSW will assist in creating an environment of learning and role model professional development.

Essential Functions

Completes biopsychosocial assessments that guide practice and are repeated throughout the episode of care. Provide counseling, brief therapies, mediation and psychoeducation to patients and their circles of support addressing a variety of needs and situations including increasing coping skills, hospice and palliative care philosophy, nonpharmacological symptom management strategies, and advance care planning. Identify and link patients and families with community resources. Document social work activities in the clinical record per Hospice standards. Participate in interdisciplinary team meetings, staff meetings, ethics consultations and other meetings as requested. Work collaboratively as a member of the Interdisciplinary Team(s). Participate in taking on-call responsibilities as necessary including weekends and evenings. Adhere to practice of confidentiality regarding patients, families, staff, and the organization. Maintain quality and productivity standards. Supervise Master level field placement students when applicable. Participate in providing support to staff and volunteers in coping with compassion fatigue. Demonstrate understanding and adherence to professional Codes of Ethics and related organizational policies. Participate in the orientation of volunteers and staff as requested. Performs all duties as assigned by the Manager or Designee. Provide clinical oversight for the MSW program under the supervision of the Manager. Facilitate MSW Meetings/responsible for providing clinical content. Provide for hourly LICSW Candidate Supervision for Hospice and SVH MSWs. Manage MSW Intern Program including mentor assignments as approved by manager. Provide orientation and support for new MSWs.

Education

Master of Social Work (MSW) degree required from an accredited program.

Experience/Training

Minimum one (1) year of professional paid social work experience a hospice program required.

License/Certification

Current Washington State LICSW Social Work licensure Meet Washington State Approved Supervisor Standards for supervising LICSW candidates. Current Washington State Driver’s License and Automobile Insurance is also required.

Other Qualifications

Excellent interpersonal, verbal and written communication skills required. Ability to practice collaboratively. Must be able to work in a fast-paced, dynamic environment characterized by frequent interruptions and competing priorities. Proven leadership skills including team development. Proven ability to intervene across a broad range of family, group and individual dynamics. Demonstrated knowledge of and commitment to social work values and adherence to the NASW Code of Ethics and Standards of Cultural Competence in Social Work. Knowledge of health care regulations, laws, ethics and policies related to applicable healthcare setting. Knowledge of needs assessment, resource identification, acquisition and community resources. Experience in utilizing a variety of appropriate social work therapeutic interventions depending on the assessed needs. Experience in mentoring and coaching preferred.
