As an LPN Clinical Field Staff Liaison (CFSL), the work you do every day makes a difference in the lives of our patients by ensuring quality and safe delivery of home health services.

HOW YOU’LL WORK:
You’ll provide support and act as a resource to clinicians in the field. You will assist in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management.
MAJOR AREAS OF RESPONSIBILITY:
Operations: Serve as a resource for field staff pertaining to clinical issues and/or concerns. Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager.
Patient Care: Work in the field with staff and patients as needed.
Customer Service: Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
Minimize Patient Risk: Focus on Outcome Monitors as assigned.
Policies: Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol.
Teamwork: Assist with monthly Case Manager, Nurse and Therapy meetings. Participate in all on-call requirements, and patient staffing as needed.
HARD & SOFT SKILLS:
Compassionate communicator with a positive attitude.
Patience is a virtue when working with patients, families, physicians, and coworkers.
Attention to detail is critical, as is being observant and following directions.
Critical thinker and the ability to problem solve clinical needs.
REQUIREMENTS:
Current LPN license in the state of employment
Minimum of Two (2) years of experience as an LPN in an acute care setting
Participates in on-call, evening/weekend shifts and provides patient care, per agency needs
Minimum one-year home health experience is preferred
Valid driver’s license and auto insurance in your name as a driver
Capable of all physical demands
