The Complex Care Case Manager will help people who are experiencing homelessness and recently discharged from a behavioral health facility navigate through a maze of intimidating options to develop a plan of action that will meet their specific needs including finding housing. You can make an important difference.

Position: Complex-Care Case Manager
Supervised By: Shelter Manager
Position Summary:
Complex-care Case Manager helps up to 15 shelter guests navigate through an integration of medical and behavioral health services during the first few weeks out of treatment services. This includes navigating transportation services and coordinating appointments. The Complex-care Navigator is an advocate for each guest.
The shelter also has a “Service Navigator” to help with services more directly connected with housing.
Each year more than 250 guests exit the shelter to a place they can call home – and keep as their home. HOPE recidivism rate remains around 5% while most shelters are above 34%. As a Complex-Care Case Manager, you will learn how to contribute to these results, consistently.
Skills: good interpersonal and communication skills, ability to use “Profiler,” self-starter, prior knowledge of community based behavioral health care case management (preferred),
Minimum Qualifications/Requirements
MSW
Certificates, Licenses, Registrations
· Valid driver’s license
· Maintain current CPR/AED certification (training available)
· Willingness to be trained in de-escalation techniques and administration of Naloxone
· Successful completion of a six-month on-the-job orientation period

Pay: $20.00 – $22.00 per year
