Mental Health Crisis Case Manager – Summary
NorthKey Community Care is seeking a full-time Mental Health Crisis Case Manager to engage with individuals and families to ensure clients are receiving the most effective and appropriate care in managing their behavioral health needs, especially in crisis situations. This position will be part of a multi-disciplinary Mobile Crisis Intervention team.

Mental Health Crisis Case Manager – Qualifications

Bachelor’s degree in a behavioral health and/or human services field of study from an accredited school is required.
At least one year experience working with individuals with a history of mental illness and/or substance use.
Relevant education, training, experience, and knowledge in the behavioral health field.
Excellent written and verbal communication skills.
A valid driver’s license and the ability to operate a motor vehicle.

Mental Health Crisis Case Manager – Job Duties

Support and connect with individuals with Serious Mental Illness (SMI) and Serious Emotional Disturbance (SED) that are experiencing a mental health crisis in order to immediately address their mental health and substance use needs.
Answer calls on crisis hotline to provide support for callers who may be at risk of suicide or homicide.
Respond telephonically to crisis callers to assess level of risk, create safety plans, and coordinate appropriate level of care response.
Collaborate with behavioral health therapists, hospitals, family members, and external sources to coordinate crisis care for crisis callers and ensure follow-up care is provide.
Engage clients in a structured, collaborative relationship which empowers them to gain necessary skills in a variety of areas related to recovery and self-management of their mental health and substance use.
Promote environmental and psycho-social health, and maximize independence to improve and maintain lifelong well-being and functional status.
Provide evidence-based care concerning complex cases of vulnerable individuals with serious mental illness, serious emotional disturbance, and chronic substance addiction.
Identify and address risks and current or potential gaps in care in order to prevent unnecessary deterioration that could result in hospitalizations, emergency room visits, and/or loss of independence in vocational, academic, and/or social functioning.
Provide education to clients to promote improved functioning skills and development.
Complete requests for prior authorizations as indicated to provide necessary services.
Participate in staffing meetings and consultation with other clinicians to assess care coordination needs of clients and encourage appropriate referrals to internal and external community services.
Act as an advocate to clients to ensure they receive necessary services in the community.
Develop knowledge and establish good working relationships with community support resources and services within the community.
Maintain communication and positive multi-disciplinary working relationships with clinicians and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations.
Complete timely and accurate documentation within an electronic health record and other documentation as required of the position.
Attend and participate in relevant continuing education and in-service training to improve professional competency and clinical intervention skills.
Maintain flexible hours to meet organizational and client needs.
